Overview
Assistant Manager / Manager, Specialized Case Management Programme (SCMP) and TAB (caregiver respite).
Plan, develop and review the services and operations of the SCMP and TAB programmes, review the post-18 service continuum, and co-lead or develop initiatives within the adult service landscape or for SPD, to meet emerging needs of clients and caregivers, or for SPD strategic development.
Responsibilities
Administration
Responsible for all matters pertaining to administration, operations and control of the SCMP/TAB programmes.
Recommend policies and practices for the efficient and effective operation and management of services under your purview and implement those policies and practices.
Operations
Drive outreach activities to enroll persons with disabilities (PwDs) and caregivers.
Develop and implement a suite of SCMP/TAB programmes to engage PwDs and caregivers.
Supervise a team including social workers and administrative staff; collaborate with government agencies and community-based organisations to offer services to adults with disabilities.
Lead the team in developing the service model / reviewing, setting service guidelines/standards and monitoring service delivery of programmes.
Responsible for achieving key performance indicators, providing progress reports and assisting in data analysis for planning and reporting purposes.
Keep abreast of latest developments in service delivery to ensure updated client services.
Submit indicators to funding agencies such as MSF and SG Enable; coordinate claims for submission to funding agencies.
Prepare annual budget for the centre and relevant papers where required.
Service Development & Sustainability
Engage in organizational planning (strategic planning, programme planning, and services review).
Ensure the sustainability of the programmes, including funding exploration and renewals.
Keep current with the funding and staffing model for the programme.
Prepare annual budget for the programme (SCMP/TAB).
Networking
Responsible for outreach to referral agencies, community organizations and hospitals for client referrals to SCMP/TAB.
Develop strong networks with other Social Service Agencies (SSAs), community organizations and funding bodies like MSF, NCSS and SG Enable towards active collaborations.
Other Duties
Any other duties as may be assigned from time to time.
Requirements
Master’s or bachelor’s degree in any discipline related to social work, or health science from a good university.
At least 5 years of relevant working experience in managing programmes for persons with disabilities.
Excellent management and leadership skills with demonstrated ability to lead a team.
Communicative, persuasive and able to encourage and motivate the team.
A good understanding and knowledge of the sector and dynamics.
