Job Title: Program Coordinator
We are seeking a skilled and experienced Program Coordinator to join our team.
In this role, you will be responsible for planning, implementing, and evaluating programs for centre-based and home care.
Key Responsibilities:
- Assist in the development and implementation of programs that meet the needs of clients.
- Work closely with the Care Team to ensure effective program delivery.
- Coordinate volunteer groups and third-parties into the programs.
- Educate the Care Team on new programs and initiatives.
- Align rehabilitation activities with care plans.
- Monitor client progress and make recommendations for improvement.
Requirements:
- Degree in Social Work, Psychology or related field.
- Experience working in a similar role.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of geriatric care principles and practices.
