Roles & Responsibilities 1.
Job Functions
- Conduct pre-emplacement interviews with residents to understand their needs, work with them to set goals, develop case plans to address concerns
- Implement, review and modify case plans
- Prepare and maintain Individualised Service Plans (ISPs), progress and closure reports
- Provide casework and counselling services to residents and their families
- Refer residents and their families to appropriate agencies for further intervention
- Ensure that the halfway house's Programme meets the mission, objectives and procedures of case management services

Candidate Requirements
- At least a Degree or Post Graduate Diploma in Social Work or related disciplines from recognised universities
- Registered as an Accredited Social Worker or Social Service Practitioner will be an advantage
- At least 2-3 years of relevant casework and counselling experience in the field, preferably working with offenders
- Good interpersonal and communication skills (oral and written)
- Able to work independently and at the same time a team player
- Self-motivated and takes initiative to drive improvements and developments
- Proficient in Microsoft Office applications such as Word and Excel, as well as able to perform basic statistical analysis

Working Hours
5 days work week
Monday to Friday - 9am to 6pm

Halfway House: 50 Lor 34 Geylang, Singapore (Catered to Male residents)
