Principales Fonctions Et Responsabilités : 

  • Fournir un soutien médical aux patients et aux fournisseurs de soins de santé, incluant, sans s’y limiter, la formation sur les maladies et les traitements précis et la gestion thérapeutique;
  • Offrir un soutien téléphonique (y compris des appels de maintien et de soutien à l’adhésion, entrants et sortants, selon les besoins);
  • Gérer la base de données et les audits réguliers des dossiers et des informations saisies.
  • ‌Administrer et respecter les protocoles de gestion de cas, y compris la surveillance clinique, la collecte de données, le suivi continu des patients, la conformité, la surveillance de l’interruption ou de l’abandon de traitement, la déclaration des événements indésirables et la communication avec les principaux fournisseurs de soins de santé;
  • Se préparer pour des séances de formation exhaustives et spécialisées sur certains traitements offertes par l’équipe pharmaceutique et y assister;
  • Travailler directement avec le spécialiste du remboursement et le médecin pour obtenir le financement des traitements du patient; participer à la recherche d’autres sources de financement au besoin;
  • Assister aux rencontres avec les représentants des commerciaux de l’industrie pharmaceutique au besoin;
  • Assister aux rencontres avec les patients, à des conférences au besoin pour éduquer les patients et les fournisseurs au sujet de la maladie ou du produit et des services de gestion de cas;
  • Travailler avec les associations de patients et les groupes de défense des droits;
  • Former et entretenir des partenariats avec l’équipe pharmaceutique;
  • Assurer la liaison avec les fournisseurs de soins de santé, les pharmacies, les cliniques de perfusion et d’autres organismes, le cas échéant, pour coordonner les perfusions et la livraison des médicaments et résoudre les problèmes au fur et à mesure qu’ils se présentent;
  • Communiquer clairement avec le gestionnaire, l’équipe et le client, le cas échéant, en ce qui concerne la gestion des cas des patients;
  • Participer avec les gestionnaires au processus de formation des nouvelles infirmières gestionnaires de cas, incluant, sans s’y limiter, l’observation au travail et la surveillance des appels;
  • Apporter un soutien et collaborer avec d’autres membres du personnel à l’élaboration et à la mise en œuvre de programmes de gestion des maladies, notamment en déterminant les exigences cliniques, en rédigeant des textes, des manuels ou des brochures pour les patients, et en élaborant des paramètres de programme de conformité, des évaluations des besoins et des protocoles;
  • Fournir des rétroactions concernant les protocoles, les bases de données, les processus et les relations interdépartementales;
  • Être disponible en dehors des heures de travail et les fins de semaine (en tout temps), pour des consultations téléphoniques et par courriel, selon les besoins du programme;
  • Être responsable de la déclaration de tous les événements indésirables aux unités/départements concernés.
  • L’infirmière/infirmier gestionnaire de cas se verra également attribuer d’autres fonctions et tâches au besoin, de temps à autre.

  • Primary Duties and Responsibilities:

  • Provide medical support to patients and health care providers, including but not limited to education on specific diseases and therapies and disease management;
  • Telephone support (including maintenance calls and adherence support both inbound and outbound as needed);
  • Database management and ongoing auditing of files and information captured;
  • Administer and adhere to case-management protocols, including clinical monitoring, data collection, on-going patient follow-up, compliance, therapy disruption/discontinuation monitoring, adverse event reporting and liaison with primary healthcare providers;
  • Prepare for and attend extensive, specialized training sessions for specific therapies provided by the pharmaceutical team;
  • Work directly with the reimbursement specialist and the physician to secure funding for their patient’s therapies; assist with alternative funding searches when needed;
  • Attend pharmaceutical sales meetings as required;
  • Attend patient meetings, conferences as needed to educate patients, providers regarding disease/product information and case management services;
  • Work with patient associations and advocacy groups;
  • Establish and maintain partnerships with pharmaceutical team;
  • Liaise with health care providers, pharmacies and infusion clinics, and other agencies as required, to coordinate infusions, drug delivery and resolve issues as they arise;
  • Clear communication to the manager, team and the client regarding the management of patient cases;
  • With the managers, participate in the training process of new nurse case managers, including but not limited to shadowing & call monitoring.
  • Support and collaborate with other program staff in the development and implementation of disease management programs including clinical requirements, write texts, manuals or patient pamphlets, develop compliance program parameters, needs assessments and protocols;
  • Provide feedback regarding current protocols, databases, processes, inter-departmental relationships
  • Be available on call after hours and weekends (24/7) for periodic telephone consultation and emails as needed by the specific program
  • Responsible for reporting all Adverse Events to the assigned units/departments.
  • The Nurse Case Manager will also be assigned other duties and tasks as required from time to time.May be required to be on standby/ on call as part of this role.
  • EXPÉRIENCE ET NIVEAU D’ÉDUCATION REQUIS :

  • Baccalauréat ou un diplôme d’infirmière autorisée/infirmier autorisé (IA), diplôme avancé ou expérience spécifique en gestion de la pratique avec cinq ans d’expérience en soins infirmiers
  • Inscription à l’Ordre des infirmiers et infirmières de la province d’emploi obligatoire
  • Au moins deux ans d’expérience dans le domaine du remboursement par un tiers public ou privé ou dans le domaine des ventes dans l’industrie pharmaceutique (un atout), des soins gérés ou du soutien clinique, ou dans un domaine connexe;
  • Le candidat idéal est une infirmière ou un infirmier avec une expérience des soins de santé et de la gestion de cas dans un milieu médical ou hospitalier, dans le domaine de l’assurance ou dans l’industrie biopharmaceutique;
  • Bilinguisme (français et anglais) fortementrecommandé; 
  • Expérience dans le domaine de l’hématologie ou de lanéphrologie un atout;

  • Experience and Educational Requirements:

  • A Bachelor’s degree or registered nurse (RN) diploma, advanced degree or specific practice management experience preferred, with five (5) years of Nursing experience
  • Registration with the College of Nurses in the province of employment is required
  • Requires a minimum of two (2) years in public or private third party reimbursement arena or pharmaceutical industry in sales (not required, but an asset), managed care, or clinical support, or related experience;
  • The preferred candidate is a nurse with healthcare and case management experience in medical or hospital practice or the insurance or bio-pharma industry;
  • Bilingualism (French and English) strongly preferred;
  • Experience in hematology and/or nephrology an asset;
