Job description

This Job Role is being offered by Tsao Foundation.


They are open to hire Seniors who are aged 50 and above.


Please note that this position does not offer work visa sponsorship.


To coordinate and operationalise the Integrated Community Care Provider (ICCP) initiative under Community Successfully Aging (ComSA) in Whampoa area.

The role ensures the effective implementation, monitoring, and reporting of ICCP activities in alignment with AIC’s deliverables.

It facilitates collaboration and communication among ICCP partners, ComSA’s programmes, and relevant community stakeholders to strengthen cross-sectoral integration and deliver holistic care for seniors, as well as to support other initiatives relevant to integrated community care.


Programme Coordination and Implementation
Coordinate and operationalise the ICCP workplan, ensuring alignment with ComSA’s integrated care strategy and AIC’s deliverables.


Collaborate with ICCP partner organisations as well as ComSA’s internal units — Community Development (CD), EPICC, and Primary Care Clinic (PCC) — to implement joint workflows, referral pathways, & collaborative initiatives.


Monitor project milestones, budgets, & deliverables to ensure progress and quality outcomes; identify risks or challenges early & work collaboratively to resolve them.


Support the Deputy Dir, ComSA, in tracking implementation progress and ensuring ICCP objectives, outcomes, and KPIs are achieved within established timelines.


Secretariat and Administrative Support
Provide secretariat function for meetings, including the preparation of agendas, documentation of minutes, and follow-up on decisions and action points.


Maintain accurate and up-to-date shared systems for documentation, data management, and reporting across ComSA units and ICCP partners.


Compile, verify, and consolidate data and financial information for ICCP progress and claim reports.


Ensure accuracy and timeliness in the submission of endorsed reports and claims to AIC and ComSA in accordance with stipulated requirements.


Monitoring, Reporting, and Evaluation
Track programme indicators and performance metrics to assess progress towards ICCP and ComSA targets.


Prepare analytical reports, dashboards, and presentations for internal management review and submission to AIC.


Support evaluation exercises and document lessons learnt to enhance integrated care practices, operational processes, and future programme planning.


Partnership and Stakeholder Engagement
Serve as the key liaison between AIC, ICCP partners, and ComSA units, AIC, and, facilitating effective communication, coordination, and implementation.


Organise and facilitate cross-partner meetings, dialogues, and review sessions to strengthen collaboration and shared accountability.


Capacity Building and Professional Development
Participate in relevant AIC training programmes to enhance competencies in integrated care, programme coordination, and partnership management.


Contribute to knowledge-sharing within the team to strengthen x-program integration
Other Duties
Undertake any other duties or special projects as assigned by the Deputy Dir in alignment with ICCP objectives and organisational priorities.


Educational Qualifications
Degree in any discipline; business admin, social science, social work & healthcare management is preferred.


Diploma with relevant experience in project or programme coordination within the healthcare or social service sector may be considered.


Skills
Min 2 to 4 years of relevant experience in programme or project coordination, administration, or partnership management within the social service sector.


Experience working with multi-agency or community partnerships (e.g., AIC, healthcare providers, grassroots organisations, or VWOs).


Proven ability to coordinate service workflows, monitor deliverables, and prepare progress and financial reports for funders or partner agencies.


Prior exposure to social services, eldercare, or integrated care initiatives will be an advantage.


Familiarity with AIC reporting systems, funding requirements, or community-based programme frameworks is desirable.


Proficient in MS Office and comfortable using cloud-based collaboration and reporting tools (e.g., SharePoint, Teams).


Strong coordination, administrative, and secretariat abilities, including minute-taking, report consolidation, and claim preparation.


Strong organisational and time-management skills; able to manage multiple workstreams and meet deadlines independently.


Basic understanding of budget tracking, data management, and programme monitoring.


Good interpersonal and communication skills, with the ability to engage diverse stakeholders respectfully and effectively.


Good working knowledge of social service operations and cross-sector collaboration.
