Job Functions
Provide case management support and counselling services to residents and their families
Plan, review and conduct rehabilitative programmes for residents
Prepare, maintain case files, prepare Individualised Service Plans (ISPs), progress and closure reports
Refer residents and their families to appropriate agencies for further intervention
Analyse trends and emerging needs of counselling and case management services
Direct and oversee the planning, development and implementation of case management services and projects
Develop and implement outreach and communication plans for halfway house’s services
Provide supervision, guidance and advice to social workers and social worker aides
Conduct regular meetings with team members regarding the service delivery and standards
Ensure that the halfway house’s Programme meets the mission, objectives and procedures of case management services
Candidate Requirements
At least a Degree in Social Work, Psychology or Sociology from recognised universities
Registered as an Accredited Social Worker or Social Service Practitioner will be an advantage
Minimum 5 years of relevant social work experience in the field, preferably working with offenders, and with at least 2 years of supervisory experience
Working Hours
5 days work week
Monday to Friday - 9am to 6pm

Halfway House: 50 Lor 34 Geylang, Singapore (Catered to Male residents)
