Job Description
You will support Inpatient / ICU / Trauma / A&E / Outpatient care by assessing psycho-social and financial needs, and providing counselling to patients including their families.
Your job scope includes close collaborations with the multi-disciplinary team in clinical setting and recommend appropriate interventions to support / facilitate a holistic care plan for patients to successfully integrate back home and into the community.
You will conduct financial assessment, field visit, build good rapport with government / community / social agencies or individuals to mobilize social resources to meet patient's needs.
You will also be expected to initiate and implement support programs / improvement projects / learning initiatives for the Medical Social Service Department.
