The Senior/Medical Social Worker will work closely with the Programme Leads to develop and provide interventions to patients and families who require support regards to treatment adherence.
Job Responsibility:
- Work alongside with programme leads to develop interventions, which includes engaging and working with stakeholders
- Conduct qualitative interviews and focus group discussions with relevant stakeholders, where applicable
- Develop, refine and provide pilot social work intervention comprising an assessment framework to facilitate the identification of areas that require support, individualised health and well-being plan as well as intensive case management
- Assist with the feasibility and evaluation studies
- Assist in preparing progress and final report of the project and cases
- Maintain the highest standard of professional conduct and record keeping in accordance with policies and procedures
- Assist with any other duties as assigned by HOD, programme lead and supervisors, as when required
Job Requirements:
- Degree in Social Work or Post graduate Degree in Social work
- Registered Social worker with the Social Work Accreditation Board (SWAB)
- At least 3-5 year(s) of direct social work experience practice (especially in working with families with young children)
- A fast and efficient team player with the ability to multi-task
- Good Communication and Written skills
