Key Responsibilities:

- Accurate documentation of case notes.
- Facilitating referrals to relevant service providers.
- Active participation in case discussions with external partners.
- Providing expert case management services.
- Conducting thorough needs assessments within the community.
- Preparing data submissions to regulatory bodies.
Requirements:
- Prior experience in social work or a related field is highly desirable.
- An undergraduate degree in Social Work is preferred.
