Job Summary: To support the Care and Counselling Department in various capacities, focusing on case management, administrative duties, and facilitating support groups.
Responsibilities: - Clinical and Case Management:
- Assist with case management after cases are screened by experienced social workers.
- Follow up with care arrangements and post-discharge referrals.
- Explain long-term care schemes to patients and their next of kin.
- Conduct home visits as instructed by social workers.
- Manage cases referred from the AMKH Day Rehab Centre for assistance with applications.
- Facilitate means testing for inpatient care.
- Accompany patients for admission assessments or centre-based services.
- Professional Development:
- Engage in continuous learning through regular case discussions, supervision, and training opportunities.
- Programme Development:
- Assist in preparing and delivering support groups.
- Administrative Support:
- Maintain accurate documentation of case files and referrals.
- Collect and submit regular statistics and reports.
- Support administrative duties when required.
- Oversee the maintenance of the C&C Resource library and brochures.
- Prepare new admissions by accessing relevant portals.
- Perform other administrative tasks assigned by the supervisor.
Requirements: - Diploma in Social Work or WSQ Diploma in Social Services.
- Fresh graduates welcome and encouraged to apply.
- At least 4 years of experience in the social service sector.
- Proficiency in Microsoft Office applications.
- Strong communication and written skills.
